Contract for UURC Intervention Services

The participation of my child _______________________________ in the UURC program is a privilege rather than a right. To ensure my child’s timely and satisfactory progress as a reader, to ensure that other UURC clients receive maximum benefits, and to facilitate efficient implementation of UURC services for the maximum number of children, I understand and agree to the following terms and conditions for my child’s participation in the program, which may include up to 90 sessions.

  1. My child will arrive on time, stay for the entire session, and depart on time in my care, or in the care of a responsible adult that I will designate. The names of all designated adults must be provided in writing to the UURC.
  2. I will sign my child "in" upon arrival and "out" upon departure. I will report the correct time of arrival and departure.
  3. I understand that I will be charged for each unexcused absence. Moreover, if my child has 3 unexcused absences, UURC services will be discontinued.
    1. Arriving 10 or more minutes late for a tutoring session constitutes an unexcused absence. Furthermore, I understand my child may not be tutored that day.
    2. If my child needs to miss a session, I will contact the UURC at least 6 hours in advance to advise them of such. Failure to contact the UURC at least 6 hours in advance constitutes an unexcused absence.
    3. If my child has been sick with a contagious illness, or has experienced fever, vomiting, or diarrhea in the 24 hours prior to a tutoring session at the UURC, I will contact the UURC at least 6 hours in advance (see #3b in full) if possible to cancel the session. I understand that if my child acquires 3 excused absences due to illness for which prior notice (6 hours) was not given, subsequent excused absences will require a doctor's note. Failure to provide said note will constitute an unexcused absence.
    4. After 10 excused absences (no charge), I will be charged for each subsequent excused absence.
  4. I understand that my child needs to be picked up immediately upon completion of his/her lesson. Three late pick-ups will result in discontinuation of UURC services.
  5. I understand that diligent effort and respectful behavior on my child's part are essential. Failure on my child's part to meet these expectations will result in discontinuation of services.
  6. I understand that discontinuation of UURC services due to failure on my part to meet the terms of this contract may preclude my child from receiving UURC services in the future.
  7. I understand that UURC staff may recommend that my child engage in reading activities at home as part of intervention. I agree to support those activities to the best of my ability.
  8. I understand that the UURC does not have waiting area facilities that are suitable for children. I understand that if I have other children with me when I drop off my child for his/her session, I will not be allowed to wait in the waiting area.
  9. I understand that tuition is due the first tutoring session of the month to be paid in full for the upcoming month. Failure to pay by the 3rd session will result in discontinuation of services.
  10. I give consent for my child to be photographed or filmed for UURC purposes.*
  11. I give permission for my child's work to be used for UURC purposes.*
  12. I give permission for the UURC to notify my state legislators that we are receiving UURC services.
  13. In case of emergency, a meeting place has been established in front of the USU Atrium building in the East parking lot.
