In order to receive credit for a course in which you have registered, download and fill out all sections of this general registration form. This form is designed to be completed and signed digitally, and must be submitted along with your course payment to earn either credit/no credit or letter grade credits from the University of Utah. Once you have registered for your course and paid the course fee, please email the completed copy of this form to sarah.kocherhans@utah.edu.
